Congratulations to Our PTN Graduates!

When practices engage in the Practice Transformation Network (PTN) grant, they agree to set aims, use data to drive care, achieve progress on the aims, achieve benchmark status for nationally recognized quality measures, and learn to thrive in value-based payment models.

These practices focus on patient engagement, community partners, coordination of care, enhanced access, continuous quality improvement, internal engagement, teamwork, and promoting joy in the work place. Our PTN practices meet with their Community Care of The Lower Cape Fear (CCLCF) coaches at least once a month to maintain momentum in quality improvement and practice transformation. The practices complete assessments every 6 months to measure progress on national goals and benchmarks. PTN graduation for our primary care practices entails meeting 27 milestones and sustaining improvement.

CCLCF would like to congratulate two of our PTN practices who have graduated! Congratulations to Dr. Melvin Gerald and Dr. Umar Bowers and the staff of G&G Healthcare, Whiteville and Cerro Gordo offices; and Dr. Dorothy Mosley and the staff at Ave Maria Family Practice in Wilmington.
