Access East is an Affiliate of ECU Health.  CCLCF provides care coordination, complex care management, and provider support services to local contracted primary care providers and their members in our six-county region.

Our objectives at CCLCF include improving health outcomes and reducing care costs. Promoting improvements in care quality including supporting the Medical Home and supporting patients through the continuum of care.

Carolina Complete Health

Carolina Complete Health, one of the 5 Prepaid Health Plans (PHP) for NC Medicaid Beneficiaries, is the only Provider Led Entity and a venture partnership between Centene Corporation and the NC Medical Society.  The CCLCF Care Team provides Care Coordination, Complex Care Management and Long-Term Support Services (LTSS) Care Management to Carolina Complete Health members in Pender, New Hanover, Brunswick, Columbus and Bladen Counties.  Services also include:

  • Services are offered to members with Social Determinants Of Health (SDOH) needs.
  • Care Managers provide face-to-face visits with Long Term Support Services (LTSS) members

Patient Rights in English and Spanish

While getting services from Community Care of the Lower Cape Fear, you have the rights listed below.

Personal Rights:

  1. Be treated with courtesy and respect by our staff
  2. Be supported by our staff as you work with your provider to decide your care
  3. Get information that is easy to understand
  4. Not be treated differently or otherwise discriminated against on the basis of race, color, national origin, sex, disability or other protected status

Your Health Information:

  1. How we keep your personal and medical information private and safe
  2. Who can get information that you give to our network

Our Programs and Services:

  1. Ask questions about our programs and services
  2. Know about contracts we have with any other companies
  3. Decide you don’t want or would like to stop our services
  4. Be informed about all care management services, even those not paid for by your insurance
  5. Talk with your medical providers about all options for care management services

Our Staff:

  1. How to change care managers
  2. The qualifications and experience of our staff
  3. The names of staff who will be part of your care management team


  1. How to file complaints with Community Care of Lower Cape Fear
  2. Know how much time it should take for someone to talk with you about your
    complaint and much time it should take for your complaint to be settled

You also have the responsibility to:

  1. Make a care management plan with our staff that you can agree to follow
  2. Tell a care management team member if you cannot follow the plan
  3. Give staff information needed to provide you services
  4. Let our staff and your medical provider know if you decide to stop our services
