About CCLCF
The Community Care of Lower Cape Fear (CCLCF) Community Care Team supports patients and providers using innovative local health care solutions to deliver whole health care in the communities we serve. Together with our partners, we are changing the healthcare experience for North Carolinians. With over 20 years of experience providing care management, we use a proven strategy to drive healthcare quality improvement and cost reduction in southeastern North Carolina. CCLCF is an independent, 501(c)(3) nonprofit and a National Committee for Quality Assurance (NCQA) accredited organization committed to ensuring patients are knowledgeable about their health and can take preventative measures to boost their wellness where possible. We strive to ensure access to health care with a special focus on hard-to-reach populations facing difficulties in obtaining care. We meet patients where they are and address barriers to healthier outcomes, including connecting them to resources for food, transportation, housing, and support for interpersonal violence or toxic stress. CCLCF staff participates in local community meetings, coalitions, panel discussions, and outreach events so we can continue understanding what our communities need and what resources we can provide to help.
In some cases, CCLCF staff work with medical providers and their patients collectively to ensure timely delivery of care through annual wellness visits, identify and managing chronic illnesses to improve patient outcomes and satisfaction. The medical provider and their clinical staff are supported by CCLCF staff to assist in improving patient outcomes, increase revenue streams with expanded support. When CCLCF staff has a presence in key medical practices we can support and enhance communication, the referral process, follow-up with patients and community resources as well as provide consultation within the clinic setting.
CCLCF’s home office is in Wilmington, North Carolina, and the CCLCF staff serves six counties, including Bladen, Brunswick, Columbus, New Hanover, Onslow, and Pender. CCLCF senior leadership and staff have years of experience working alongside our community hospitals, community organizations, and medical practices and are well versed in community resources to address food insecurity, medical transportation, housing, and support for interpersonal violence or toxic stress. CCLCF as a clinical health care team has over 20 years of experience and participates in local community meetings, coalitions, panel discussions, and other outreach events to ensure comprehensive understanding of the medical needs within the service area.
