Our care management team strived to meet patients where they were and helped to remove barriers to healthier outcomes such as food instability, housing concerns, or access to transportation. Community Care of the Lower Cape Fear has joined CAPE FEAR HEALTHNET INC in managing their member’s chronic conditions and navigating different areas of the health system. Cape Fear HealthNet was awarded a generous grant from The Leon Levine Foundation, bringing CCLCF’s care coordination in to support specialty referral follow-up.

Voyage

CCLCF has partnered with Voyage (formally Blue Ribbon Commission) to expand support to Voyage’s children and families. Voyage’s team of Community Outreach Advocates serve a vital role at D.C. Virgo acting as a liaison between the school and the community working to meet the significant needs of the student population. Voyage is moving to expand its reach into other schools and communities in need. CCLCF’s Care Team’s and Voyage’s Community Outreach Advocates will work together to build on Voyage’s public health and community-based approach adding health care education, connection to health resources, and CCLCF care management services to Carolina Access Medicaid children and families. Dr. Megan Rose, CCLCF Deputy Director and Network Pharmacist, sits on the Voyage Northside Health and Wellbeing Improvement Team.

CCLCF has partnered with Carolina Complete Health for a pre Medicaid Transformation Go-Live pilot. The purpose of the P4P pilot program (Pay for Performance Optimization Opportunity) is to help practices examine their billing and coding practices to ensure they are getting maximum reimbursement for the work being done. CCLCF’s Population Health Team and Carolina Complete Health representatives met with several providers to assess “current coding and documentation as it relates to pay-for-performance measures and to educate on best practices, help improve coding accuracy, and maximize pay-for-performance incentive opportunities so that providers are better equipped for Go-Live”.  CCLCF also partners with Carolina Complete Health to bring resiliency trainings to our community. Community Resilience Model (CRM)® trainers from both organizations work together to help create “trauma-informed” and “resiliency-informed and focused” communities that share a common understanding of the impact of trauma and chronic stress on the nervous system and how skills learned can help improve resiliency.

DocsInk

Community Care of the Lower Cape Fear has expanded its partnership with DocsInk, a local option for telehealth. We have integrated DocsInk into our care management and coordination for over a year and would like to introduce CCLCF practices to a possible telehealth solution, also allowing direct referral and communication with the CCLCF Care Team. It has provided an essential space for our care teams to share data, communicate securely with providers and videochat with patients. DocsInk is offering special pricing for providers and staff to help with virtual patient encounters, care coordination, and more. CCLCF  is offering to help network practices operationalize free of charge.

Harrelson Center

CCLCF and The Harrelson Center have partnered to bring health care navigation to the Harrelson Center Help Hub. Susan Boyd, RN BSN, Care Manager will provide clients who have health concerns with healthcare resources, education, medical referrals, and will address barriers to healthier outcomes, including connecting them to resources for food, transportation, housing, and support for interpersonal violence or toxic stress. Ms. Boyd will be available during the Help Hub’s normal business hours, Tuesday through Thursday, 9 AM until Noon to provide in-person or telehealth services beginning August 1st. The Help Hub at the Harrelson Center serves individuals and families in need with short term/emergency financial assistance, referrals to Harrelson Center non-profit partners, connection to community resources, and now, healthcare navigation.

Mt Calvary Center Lofo

CCLCF is excited to announce our membership to the Mt. Calvary Executive Leadership Society. Mt. Calvary Center for Leadership Development serves mostly low income and rural communities, including under-served minority communities. Mt. Calvary was recently awarded a COVID-19 Community Health Worker Initiative grant supported by the North Carolina Department of Health and Human Services. We are honored to join Mt. Calvary and other community partners supporting this great work.

CCLCF has partnered with Cape Fear Collective on the Career Impact Bond pilot; providing care management support for three students while they are enrolled in the LPN to RN program at Cape Fear Community College. Cape Fear Collective is currently working on the career impact bond program pilot with the help of a grant from the Kate B. Reynolds Charitable Trust.
