Continued support during COVID-19

In an effort to keep our patients, providers, and staff safe, CCLCF has been providing virtual care management and provider support until we are clear to resume in-person interactions. Though we have suspended face to face encounters with patients and providers, CCLCF continues to provide our intensive care management services with telehealth and is accepting referrals. CCLCF has integrated the most up-to-date COVID-19 screening procedures for patients who feel symptomatic or may have come in contact with someone infected. CCLCF staff will be available during our regular business hours and our Population Health Team (Provider Services) is actively reaching out to practices and providers to share information and provide support.
