Wilmington, N.C. (November 8, 2018) – Community Care of the Lower Cape Fear (CCLCF) received a 100, the highest possible score, from the National Committee for Quality Assurance (NCQA) for Three-Year Case Management, effective October 31, 2018 – October 31, 2021. It is NCQA’s highest accreditation status for service and quality that demonstrates strong performance on NCQA’s rigorous requirements for case management. It is the second time CCLCF has received accreditation for complex care management, the first time in 2016.
“NCQA Accreditation highlights organizations that are dedicated to quality improvement and deliver efficient, cost-effective case management services.” Standards include improving operational efficiencies by using evidence-based standards to perform a gap analysis and using accreditation standards to align goals and approaches to case management across payers and providers. Using this quality improvement framework ensures that those accredited implement best practices including:
- Efficient and cost-effective case management processes and service delivery
- Managing patients as they move between care settings
- Using patient-centered assessment to determine care needs
- Developing personalized, patient-centered care plans
- Providing access to qualified case management staff
- Protecting patients’ personal health information
- Patient monitoring to track care-goal progress
