Community Care of the Lower Cape Fear provides a care management team of registered nurses and social workers, behavioral health specialists, pharmacists, health care coaches, and member support specialists to help figure out your health care needs and support your provider.
Our services are provided at no cost to eligible patients.
Our goal is to improve your health.
Our care managers will help you learn how to take care of your chronic health problems, like asthma, diabetes, and heart failure.
They will work closely with you and your doctor to understand your health care needs.
The Medical Home
Your primary care doctor’s office is also known as your medical home. It is the place you should go for your health care needs. Your Medical Home offers the best care because the staff know you, your family, and your family’s health history.
You can call your medical home 24 hours a day, 7 days a week, for help or advice. Your medical home can also connect you to other doctors if you need special care.
Use your Medical Home for:
- Physical Exams and Immunizations/Shots
- Sick care needs like fever, flu, cough and sore throat
- Urgent needs like asthma flairs, infections, sprains, and cuts
Only go to the Emergency Room if you need immediate help!
- When bleeding won’t stop
- When breathing is hard
- After a serious accident
- After a seizure or convulsion
