CCLCF’s partnership with Physician Quality Partners helps generate $9.7 million in savings

Wilmington, North Carolina – Community Care of the Lower Cape Fear’s (CCLCF) partnership with Physician Quality Partners (PQP), New Hanover Regional Medical Center’s Accountable Care Organization (ACO), helped PQP generate $9.7 million in savings while earning a top quality score of 98.75 percent. Since 2016, CCLCF’s PQP care team has provided complex care management to engage and provide whole health care to Medicare beneficiaries served by PQP.

PQP made the announcement about the Medicare Shared Savings program, saying “PQP improved care for almost 20,000 Medicare beneficiaries in New Hanover and the surrounding counties, saving Medicare over $9.7 million by meeting quality and cost goals in 2019, according to recently released performance data from CMS, the federal agency that administers Medicare. PQP reduced the average cost of care by $501 per beneficiary”.

High risk beneficiaries served by PQP practices were offered CCLCF Care Team support including care management services integrated with the primary care provider, CCLCF’s pharmacy team, behavioral health specialists, health care coaches, inpatient care managers and member care coordinators who connected beneficiaries to much needed resources. Those collaborative efforts helped PQP’s hospital admission rates drop 17% with ED rates dropping 6.3% since 2016. “CCLCF has proudly contributed to PQP’s Shared Savings program since it first launched,” says Angela Ives, CCLCF Executive Director. “Our organization has worked for over 17 years to improve the health outcomes of our patients, decreasing hospital and ED utilization, and reducing health care costs in our region. We know it works and are thrilled to be part of a program that is invested in the health of our community”.
