Wilmington, North Carolina – It is with much excitement that we are pleased to announce Community Care of the Lower Cape Fear’s (CCLCF) new Medical Director, Dr. Michelle Jones. Dr. Jones joined CCLCF as Associate Medical Director in October 2020, alongside the now retired Dr. Henry Hawthorne, before her transition to Medical Director.
Dr. Jones has been a licensed physician since 1996, graduating from medical school at East Carolina University in Greenville and completing her internship and residency at Duke University. Dr Jones is Board Certified through the American Board of Family Medicine and a Fellow of the American Academy of Family Physicians. She is a practicing physician at Wilmington Health Associates and will continue her work as a family physician.
We are delighted to have had Dr. Jones join CCLCF and further advance our pursuit of transforming population health in the Cape Fear region. With her experience and knowledge, Dr. Jones has already established herself as an integral part of the CCLCF leadership team.

CCLCF is a 501(c) (3) that improves health outcomes for eligible Medicare, Medicaid, uninsured and privately insured enrollees in southeastern NC including New Hanover, Pender, Brunswick, Columbus, Onslow, and Bladen counties. Our interdisciplinary Care Teams including RN care managers, social workers, pharmacists and provider support, work with Prepaid Health Plans, ACOs, practices and providers to improve patient outcomes, experience, and satisfaction. CCLCF has a regional population health partnership with primary care providers, 7 hospitals, local health departments, social service agencies, Behavioral Health, Managed Care Organization\’s (MCO\’s) and other community-based organizations.
