CCLCF is an agency that provides services in a variety of ways under Medicare, Medicaid, and Private Insurances to ensure timely care to reduce high-cost health care. Some of these services are conducted under a team focused care management approach. Care management is a set of activities to improve patient care, reduce unnecessary need for additional medical care, avoid duplication of care, and assist patients and family members a more effective way to manage health conditions.

In some cases, CCLCF staff work with medical providers and their patients collectively to ensure timely delivery of care through annual wellness visits, identify and managing chronic illnesses to improve patient outcomes and satisfaction. The medical provider and their clinical staff are supported by CCLCF staff to assist in improving patient outcomes, increase revenue streams with expanded support. When CCLCF staff has a presence in key medical practices we can support and enhance communication, the referral process, follow-up with patients and community resources as well as provide consultation within the clinic setting.

CCLCF’s home office is in Wilmington, North Carolina, and the CCLCF staff serves six counties, including Bladen, Brunswick, Columbus, New Hanover, Onslow, and Pender. CCLCF senior leadership and staff have years of experience working alongside our community hospitals, community organizations, and medical practices and are well versed in community resources to address food insecurity, medical transportation, housing, and support for interpersonal violence or toxic stress. CCLCF as a clinical health care team has over 20 years of experience and participates in local community meetings, coalitions, panel discussions, and other outreach events to ensure comprehensive understanding of the medical needs within the service area.
