Access East is an Affiliate of ECU Health.  CCLCF provides care coordination, complex care management, and provider support services to local contracted primary care providers and their members in our six-county region.

Our objectives at CCLCF include improving health outcomes and reducing care costs. Promoting improvements in care quality including supporting the Medical Home and supporting patients through the continuum of care.

Carolina Complete Health

Carolina Complete Health, one of the 5 Prepaid Health Plans (PHP) for NC Medicaid Beneficiaries, is the only Provider Led Entity and a venture partnership between Centene Corporation and the NC Medical Society.  The CCLCF Care Team provides Care Coordination, Complex Care Management and Long-Term Support Services (LTSS) Care Management to Carolina Complete Health members in Pender, New Hanover, Brunswick, Columbus and Bladen Counties.
