Since being established in 2003, Community Care of the Lower Cape Fear (CCLCF) is an independent 501(c)(3) nonprofit and National Committee for Quality Assurance (NCQA)-accredited organization with over 20 years of experience in improving health outcomes across Southeastern North Carolina since being established in 2003. Our Community Care Team supports patients and providers through innovative, local solutions that deliver whole-person care and transform the healthcare experience for North Carolinians.

CCLCF works collaboratively with medical providers, community organizations, and human service agencies to ensure access to care, particularly for hard-to-reach populations facing barriers such as food insecurity, transportation challenges, housing instability, and toxic stress. By meeting patients where they are, we help them become active participants in their health as well as connect them to the resources needed to thrive.

Our staff supports medical practices across six counties (Bladen, Brunswick, Columbus, New Hanover, Onslow, and Pender) by assisting with care coordination, annual wellness visits, and chronic condition management to improve outcomes and satisfaction. Through our partnerships with various community clinics and organizations, CCLCF enhances communication, strengthens referral processes, ensures timely follow-up, and provides direct consultation to care teams.

As one of three Network Leads for the North Carolina Healthy Opportunities Pilot (HOP) Program, CCLCF’s Cape Fear HOP plays a vital role in integrating social determinants of health (such as food, housing, and transportation) into coordinated care delivery. Our leadership and staff remain deeply engaged in community coalitions, outreach events, and partnerships to ensure our programs align with the evolving needs of the people and providers we serve.
