We are honored to share that Community Care of the Lower Cape Fear has been awarded a $1.5 million strategic planning grant from the New Hanover Community Endowment to support the development of a Social Care Network with local Human Service organizations, building on the success of the Healthy Opportunities Pilot (HOP)!
This investment will enable us to strengthen our coordination among health and social support providers, as well as to continue improving the community health across all counties we serve.
We extend our deepest gratitude to The Endowment for this trust and support. This grant empowers us to deepen our commitment to health and community resilience throughout our region. We look forward to sharing updates on how this funding will make impacts in the months ahead!
